Insurance

 I am considered an out-of-network provider. I am happy to verify your insurance coverage, and if you have acupuncture benefits, I will submit your claim. If you would like to utilize your medical benefits, please click the link to email your insurance information. Include your name, date of birth, insurance information (front and back of your card), the subscriber's name and date of birth, and the reason for the acupuncture appointment.

 

I also accept Workers' Compensation and Personal Injury cases. Please contact the office before scheduling an appointment related to a claim.

 

​I offer a time-of-service discount as listed under the Booking page. I accept FSA/HSA cards, Visa, Mastercard, American Express, and cash. I am happy to provide a Superbill that you can submit to your insurance carrier for possible reimbursement.
