Insurance

Currently, I am in-network with only Contra Costa Health Plan (CCHP).  To verify your coverage, please click the link below and provide your name, date of birth, insurance information (front and back of your card), the subscriber's name and date of birth, and the reason for the acupuncture appointment. I will verify your insurance coverage and contact you.  

I am not contracted with Medicare and cannot submit a claim to them, even if you also have the Contra Costa Health Plan. 

For all other plans, I am considered an out-of-network provider. I am happy to verify your insurance coverage, and if you have acupuncture benefits, I will submit your claim. If you would like to utilize your medical benefits, please click the link to email your insurance information. Include your name, date of birth, insurance information (front and back of your card), the subscriber's name and date of birth, and the reason for the acupuncture appointment.

 

​I offer a time-of-service discount as listed under the Services page. I accept FSA/HSA cards, Visa, Mastercard, American Express, and cash. I am happy to provide a Superbill that you can submit to your insurance carrier for possible reimbursement.
